Tamsulosin (Flomax)  

The alpha-adrenergic blockers have one effective and strong drug known as Tamsulosin or Flomax which often used to treat symptoms of enlarged prostate (benign prostatic hyperplasia or BPH): difficulty urinating (hesitation, dribbling, weak stream, and incomplete bladder emptying), painful urination,  urinary frequency and urgency, and others. The main function of Tamsulosin is to relax all the blood vessels in order to regulate the blood flow. This drug also relaxes the muscles of the bladder region thus helping patient to urinate easily without much of the irritability or pain.  

Indications: Tamsulosin is indicated in treating symptoms of BPH (benign prostatic hyperplasia), also known as enlargement of prostate. This drug is an alpha-blocker and relaxes muscles in the prostate and bladder. Patients are advised to take this medicine to find relief from difficult urination, weak urine stream, and frequent urination. Tamsulosin is also indicated for getting rid of kidney stones through urinary passage. However, it is strongly prohibited to use the medication for treating high blood pressure.   

Dosage: For different symptoms of BPH, a simple doze of 0.4mg of Flomax (a single tablet) is prescribed. The perfect way of taking this drug is to consume it within half or one hour of having meal, everyday. In case the patient does not responding to the minimum dosage of 0.4mg, a higher dosage of 0.8 mg can be prescribed by the doctor. One single dosage of 0.8mg is to be taken daily. It must also be kept in mind that Flomax should not be given in combination with CYP3A4. If the dosage of the Flomax is discontinued for a certain period, it must be resumed with the minimum dosage of 0.4mg only. 

Contraindications: Patients who are having a higher level of sensitivity with tamsulosin, or any of its compounds, should not be prescribed the medication. The intake of Tamsulosin for hypersensitive patients may cause pruritus (unpleasant sensation or desire to scratch), hives, skin rash, and others. 

Mechanism of Action (MOA): Flomax mainly works against the alpha-1 receptors, as it is an adrenoceptor blocking drug. The medication releases three different kinds of alpha1-adrenoceptor that exhibits different characteristics for each different type of tissues. Mainly the receptors works best in the prostate area, making it an ideal drug for prostate related ailments. It is important to realize that Flomax must never be used as a relief for hypertension.  

Interactions: The main drugs with that Tamsulosin should never be used with are CYP2D6 and CYP3A4. Ketoconazole, a strong group member of the CYP3A4 family, should also be never used along with this flomax. Other drugs which must not be used in combination with this drug include paroxetine, terbinafine and cimetidine. 

Side Effects: There might be various side effects from the usage of this drug. Flomax often reduces the blood pressure to such a level that a patient experiences dizziness, headache and recurrent fainting spells. Patient may even feel the giddiness at the time of sitting or standing up. Apart from this, skin may face certain allergic reactions in the form of rashes or hives. Difficult breathing, heat blisters, prolonged and painful erection are some of the other effects that this drug may bring along.
